Mr Keith completed his specialist orthopaedic training in Victoria. He became a Fellow of the Royal Australian College of Surgeons (FRACS) in 2013 and is recognised as a Specialist Orthopaedic Surgeon.
The FRACS distinction is only awarded to specialist orthopaedic surgeons who have completed a minimum of 12 years medical and surgical education, with at least 5 years specialist postgraduate training.

You will be sent a Patient Registration Form after making your appointment. Please complete and return prior to your appointment along with:
- Your referral letter from your specialist or General Practitioner
- Any recent reports – X-rays/ultrasounds/MRI scans
(You can request these from the imaging provider) - Valid Medicare card
- Private health insurance information (if applicable)
- WorkCover, TAC, DVA claim numbers (if applicable).

IMPORTANT NOTE – Please advise where any xray, ultrasound and/or MRI’s were taken PRIOR to your appointment. For example: I-Med, MIA, Healthscan, Capital Radiology etc.
Mr Keith requires access to your images online during your appointment. Scans are NOT sent through by radiology groups. Discs are unreliable to access and may delay your appointment.
